Parent & Player Training Agreement

This Agreement is entered into between JH Elite Training (“Trainer”) and the Parent/Legal

Guardian of the participating athlete (“Player”).

By accepting, the Parent/Guardian agrees to the following terms and conditions:

1. Training Services

JH Elite Training provides private, group, team, and/or camp-based soccer training focused on

player development, skill progression, and confidence building. Training sessions may include

physical activity, drills, and competitive play.

2. Payment & Fees

All training sessions, programs, camps, and packages must be paid in full prior to participation unless otherwise agreed upon in writing.
Session rates, packages, and camp fees are listed on the JH Elite Training website or communicated directly.
By submitting the initial payment or deposit, participants are committing to their reserved spot in the program, session, or package.
All payments and deposits are non-refundable once the initial payment has been made, as spots are limited and reserved upon registration.
Missed sessions by participants will not be refunded unless otherwise approved by JH Elite Training.

3. Cancellations & Rescheduling

A minimum of 24 hours notice is required to cancel or reschedule a session.
Cancellations made with less than 24 hours notice, or no-shows, will result in forfeiture of the session.
JH Elite Training reserves the right to cancel or reschedule sessions due to weather, facility availability, illness, or unforeseen circumstances. In such cases, sessions will be rescheduled or credited.

4. Late Policy

Players arriving late will still end their session at the originally scheduled time.
Repeated lateness may result in loss of training time without refund.

5. Player Conduct

Players are expected to show respect toward coaches, teammates, facilities, and equipment.
Disruptive, disrespectful, or unsafe behavior may result in removal from a session without refund. JH Elite Training reserves the right to discontinue training if behavioral issues persist.

6. Injury & Medical Responsibility

Soccer training involves physical activity and inherent risk of injury.
Parent/Guardian confirms the Player is medically cleared to participate.
JH Elite Training is not responsible for injuries sustained during training, games, or related activities.
Parent/Guardian agrees to assume full responsibility for medical expenses.

7. Assumption of Risk & Liability Waiver

Parent/Guardian voluntarily releases and holds harmless JH Elite Training, its coaches, staff,

and affiliates from any and all claims, liabilities, or damages arising from participation in

training, including injury or property loss, except in cases of gross negligence.

8. Photo & Video Release

Parent/Guardian grants permission for JH Elite Training to use photos and videos of the Player

taken during training for promotional purposes (social media, website, marketing).

Opt out by specifically communicating with JH Elite Training.

9. No Guarantees

JH Elite Training does not guarantee team placement, playing time, scholarships, or advancement.

Player development varies based on effort, attendance, and individual progression.
